13. If you owned G-Coins or G-Dollars as of the close of trading on July 1, 2020, identify the number of G-Coins or G-Dollars you held in the box marked “ENDING HOLDINGS.” If you did not own any G-Coins or G-Dollars as of July 1, 2020, check the box marked “If none, check here”. Once completed, click ‘NEXT’.
14. Upload any and all supporting documentation, including shareholder agreements relating to the purchase of GTV Common Stock, G-Coins, or G-Dollars; copies of checks and wires reflecting payments for these securities; proof of your refund including the name of the refunding entity;  other documents relating to the purchase or sale of these securities; and, if you are submitting a claim on behalf of someone else,  power of attorney or other documents reflecting your authority to act on behalf of the claimant, by clicking ‘Choose File’, then click ‘NEXT’.
15. Fill out the online payment election form and indicate whether a potential payment should be issued either in the form of a check or the form of a wire. Click ‘NEXT’ when you’re finished.
16. Read Part III (CERTIFICATION AND SIGNATURE) and electronically sign the Certification at the bottom of the page (if signing on behalf of the claimant, please indicate in what capacity you are signing on behalf of the claimant), then click ‘SUBMIT’.
